What is an independent medical examination (IME)?

The short definition

An independent medical examination is a single, non-treating evaluation carried out by a qualified health practitioner to answer specific medical questions in an insurance or legal matter. The examiner has no ongoing relationship with the patient, which is what makes the opinion “independent.” The output is not a prescription or a treatment plan — it is a written report that addresses diagnosis, causation, the reasonableness of past treatment, future care needs, work restrictions, and any permanent impairment.

Who requests an IME, and why

What happens during the exam

1. Records review. Before you arrive, the examiner reads the incident report, imaging, therapy notes, and prior medical history.
2. History interview. You describe the mechanism of injury, symptom timeline, treatment so far, and how the injury affects work, sleep, and daily activities.
3. Physical examination. Range of motion, strength, neurologic and orthopedic testing appropriate to the injuries claimed. Cognitive screening is added where a concussion or brain injury is suspected.
4. Correlation with imaging. Findings on exam are compared against X-ray, MRI, or CT results to separate acute injury from pre-existing changes.
5. Written report. A narrative report answers the referring party's questions, with citations to the record and, where applicable, impairment ratings under the AMA Guides.

How to prepare

Bring photo ID, your medication list, and any imaging discs or reports you hold. Be accurate rather than dramatic: describe good days and bad days honestly, and do not exaggerate or minimize. Note the time the examination started and ended and what testing was performed — that detail is often relevant later. If a claims representative or attorney has given you instructions, follow them.

How the report affects a claim

An IME report frequently becomes the central medical document in a claim. It can support or undercut causation, justify or dispute the cost of past treatment, and set expectations for future care. A well-built report cites the record, explains its reasoning, and can be defended in deposition. A templated one usually cannot — which is why the quality of the examiner matters as much as the exam itself.
